LIPOSUCTION AND LIPOSCULPTURE
Liposuction and liposculpture are two forms of body contouring surgery, and are among the most popular and successful cosmetic procedures I perform. Liposuction removes excess fat deposits from specific areas such as the abdomen, thighs, hips, buttocks, arms, chin, and neck. Liposculpture is an evolution of liposuction that uses fine cannulas to sculpt and refine very specific, superficial areas of the body.
I use liposuction both as a standalone procedure and in combination with other surgeries such as breast reduction or a tummy tuck. These procedures help improve the overall balance and proportions of the body.
“Even with a healthy lifestyle, regular exercise, and a balanced diet, it can be difficult to achieve a body that makes you feel truly fit and confident, as stubborn areas of fat may remain.”
My patients choose body contouring for a variety of reasons. Some want to look their best in swimwear or everyday clothing, while others hope to restore the body they had before having a child. Others wish to put the finishing touch on their silhouette after completing a weight loss journey. My approach is to tailor each procedure to your unique needs, ensuring results that are beautiful, natural, and aligned with your personal goals.
UNDERSTANDING THE PROCEDURE
Liposuction involves removing unwanted fat from specific areas of the body using fine cannulas connected to a suction device. There are several types of liposuction, each designed to improve the contours and definition of your body in slightly different ways. So far, there is no clear evidence that any one technique is significantly more effective than the others.
What matters most is working with an experienced and skilled professional.
Traditional liposuction, also called tumescent liposuction or suction-assisted liposuction (SAL), remains the most effective and widely used technique, and it is the one I choose most often in my practice. The procedure begins with the injection of a tumescent solution, which numbs the tissue and minimizes bleeding. Small metal cannulas, ranging from 2 to 5 millimeters in diameter, are then connected to a suction device that generates a vacuum of approximately 0.6–0.7 bar. The cannula is moved methodically back and forth in the treatment area to loosen and then remove excess fat cells.
Power-assisted liposuction (PAL), on the other hand, uses a vibrating handpiece to which traditional cannulas are attached. This automated movement makes the procedure less physically demanding for the surgeon, but also slightly less precise than traditional liposuction. Ultrasound-assisted liposuction (UAL), or VASER, uses low-frequency ultrasound energy to emulsify the fat before it is removed through suction. This technique uses cannulas equipped with an ultrasonic transducer at the tip, which helps break down the fat tissue. I prefer to reserve this method for larger areas of liposuction, as it results in less blood loss and allows the removal of greater amounts of fat.
Meanwhile, laser-assisted liposuction uses a vibrating cannula with a laser tip that breaks down the fat cells. Since this is a fast liposuction procedure that uses heat from the laser tip under the skin, I only use it for small areas, especially in the neck, knees, or legs. Regardless of the technique, this surgery must always be performed in a fully equipped operating room, following strict sterile protocols.
“I believe that a careful and precise surgical technique makes liposuction a very safe procedure, without unexpected or unwanted complications.”
FOR A MORE CONSCIOUS CHOICE
The ideal candidate for liposuction is at or near their desired weight, in good health, a non-smoker, and committed to maintaining healthy habits after the procedure. A successful liposuction/liposculpture treatment always begins with having realistic expectations. During your consultation, we will discuss the technical aspects of the procedure, its potential benefits and risks, and the results you can expect. Liposuction is not a weight-loss method. It can refine your body contours, helping you appear slimmer and more toned, but it is not intended for significant weight reduction. The best candidates are those close to their ideal body weight, or only slightly above it, who have specific areas, such as the abdomen, flanks (“love handles”), or outer thighs (“saddle bags”), that are disproportionate to the rest of their body. This technique targets only unwanted fat and does not improve skin quality, stretch marks, or cellulite. These concerns can be addressed with less invasive treatments, often involving advanced technologies and combined regenerative treatments.
Often, patients say things like, “I have my mother’s thighs” or “I have my grandmother’s hips.” In these cases, diet and regular exercise alone may not be enough, and a surgical approach is the best option to target specific problem areas. Liposuction is particularly effective in certain parts of the body, especially the lower abdomen, flanks, and outer thighs. This procedure can greatly enhance a woman’s perception of her body shape, rebalancing proportions and boosting self-confidence. However, liposuction does not replace a healthy lifestyle or obesity treatments. In fact, patients who experience the most satisfying results are usually already happy with their weight before surgery. Body sculpting then becomes the finishing touch, used to perfect and harmonize isolated problem areas.
Liposuction is recommended for individuals in good overall health or who are slightly overweight. It is a safe and effective procedure with a relatively short recovery period. The incisions are very small, and the risk of surgical complications is low. However, depending on your work and lifestyle, it is important to carefully choose the timing of your procedure. Bruising, swelling, and mild soreness may occur for a week or longer. A supportive compression garment should be worn for 40–50 days after surgery to manage swelling, promote skin tightening, and help your body adapt to its new contours. Swelling and bruising typically fade within about 6 weeks, while the treated areas may take up to 6 months to fully settle. Most patients can return to their normal routine relatively quickly, though recovery time varies depending on the number of areas treated. In general, patients take about a week off from work and regular activities. It is essential to allow yourself sufficient time for a safe and healthy recovery. Once achieved, results can be stable and long-lasting, provided you maintain a consistent weight. Significant weight gain in the future may cause remaining fat cells in the treated areas to expand or lead to fat accumulation in other areas. The procedure is most effective when it encourages patients to continue making healthy lifestyle choices. We strongly believe that liposuction can be life-changing for patients who have worked hard to achieve a body they love.
For the first 4 weeks after surgery, you should wear a supportive compression garment both day and night. After this period, it should be worn only during the day for an additional 3 weeks. Around 3 weeks post-surgery, I recommend starting mechanical massage and lymphatic drainage treatments to stimulate lymphatic flow and improve and accelerate healing. With advanced liposuction, some bruising may occur, but it typically resolves within a couple of weeks. Gentle skin massage, a balanced diet, proper hydration, and avoiding nicotine-based products all help speed recovery and ensure the best possible outcome. Remember that the end result will not be visible immediately. During the first 72 hours, swelling will increase, but by 6 weeks, most will have resolved. Between 3 and 6 months after surgery, your body’s contours will begin to show their final shape, and after 1-2 years, the scar tissue will be fully softened.
On the day of your surgery, our team will welcome you to the operating room. After completing the preparation and pre-operative checks, you will be taken for local or general anesthesia, depending on the type and extent of the procedure. Once the surgery is finished, you will be closely monitored during recovery before being discharged home or returned to your room.
The duration of the procedure depends on the extent of the areas being treated and the technique used. A small liposuction may take around 1 hour, while more complex procedures or treatments involving multiple areas can take 2-3 hours.
Thanks to anesthesia, you will not feel any pain during the procedure. In the first few hours after surgery, you may experience some discomfort or a feeling of tightness in the treated areas, but this is generally easily managed with prescribed pain medication. Additionally, the immediate application of a shaping and compression garment over the areas where fat was removed provides comfort and relief.
Recovery varies depending on the extent of the liposuction. In general, patients can resume light daily activities within a few days, while more strenuous activities should be avoided for several weeks. Swelling and bruising gradually resolve over 2–4 weeks.
You must wear the compression garment for at least 6 weeks after surgery. The compression helps shape your body and reduce swelling and bruising. Avoid strenuous activity and sudden movements during the first few weeks. Following recommendations for hydration and a balanced diet, along with careful care of your small incision wounds, promotes healing and helps achieve the best possible aesthetic results.
Liposuction is a safe procedure when performed by an experienced surgeon, but like any surgical intervention, it carries potential risks, such as minor bleeding, bruising, swelling, temporary changes in skin sensation, and, in rare cases, irregularities in the skin surface. Choosing the right facility and a qualified surgical team helps minimize these risks.
Liposuction is not a weight-loss procedure but a body-contouring treatment. It is recommended to be at a stable weight before surgery and maintain a healthy lifestyle afterward to preserve the results over time. Our team will support you throughout the pre-operative preparation and post-operative phase, using protocols designed to optimize recovery and enhance your results.
Regular follow-up appointments will be scheduled in the weeks after surgery to monitor healing, remove any sutures if necessary, and assess both the results and their progression over time. During these visits, you will have the opportunity to ask any questions about your post-operative recovery.
The team consists of surgeon Dr. Elisa Bolletta, the anesthesiologist, operating room nurses, and post-operative support staff. They work together to ensure your safety, comfort, and the best possible aesthetic outcome.
