LABIAPLASTY AND FEMALE GENITAL FAT GRAFTING

The wide variety of shapes and sizes of the female labia makes each person unique. Yet they can make some women feel self-conscious or experience physical discomfort that keeps them from fully enjoying life. Enlarged, asymmetrical, or protruding labia can cause daily embarrassment, negatively affecting the way you dress, exercise, and experience intimacy.

Some women are bothered by the appearance or sensation of their inner labia (labia minora) when they extend or hang beyond the outer labia (labia majora). This excess tissue can cause discomfort even when wearing tight clothing or during activities such as cycling or horseback riding.

UNDERSTANDING THE PROCEDURE

Although discussing such a personal and intimate topic can feel embarrassing, there is nothing wrong with wanting to feel comfortable in your own body. I conduct evaluation consultations with the utmost sensitivity and discretion.

Some women may be good candidates for a traditional labiaplasty, which involves the simple excision of excess tissue with precise reconstruction of the labial edges.  The procedure involves removing a portion of skin along the length of the inner labia using a straight-line incision. The incision is then sutured along the edge, leaving a nearly imperceptible scar. This surgery effectively improves symmetry and reduces friction. More often, I prefer the wedge excision technique, originally described by American surgeon Gary Alter, which involves removing a V-shaped, or wedge, section of tissue from the widest part of the labia. The two sides are then sutured together, resulting in a less visible scar.

The wedge excision typically produces a more natural look.

In selected cases, to further enhance the symmetry and volume of the outer labia, autologous fat grafting (lipofilling) may be performed in combination with labiaplasty. This technique involves harvesting a small amount of fat tissue from a donor site, usually the abdomen or thighs, which is then purified by centrifugation or filtration and gently reinjected into the outer labia. The procedure restores natural fullness and softness to the tissues, improving both the aesthetic harmony and local comfort in a completely natural and biocompatible way.

FOR A MORE CONSCIOUS CHOICE

Most women who consult with us are concerned about the appearance of their labia after childbirth or as they age, since these events can cause loosening and sagging of the tissue. We also see younger women who feel self-conscious because their labia are naturally larger. If you are unhappy with your labia or experience discomfort with certain clothing or activities, labiaplasty may be the right choice for you. It is important to note that labiaplasty focuses solely on the appearance of the labia. It is not intended to restore volume to thin or sagging labia, address symptoms related to thinning of the skin and mucosa, or tighten the vagina. Situations such as reversing the effects of aging or childbirth, improving intimacy, or treating urinary leakage require a different approach, and in some cases, these procedures can be combined. Among key vaginal rejuvenation procedures, we often opt for a regenerative therapy using your own fat-derived stem cells (ADSCs). In Dr. Elisa Bolletta’s practice, this procedure has shown the best medium and long-term results. This is due to the multiple regenerative properties of stem cells, which enhance blood supply, giving the treated area renewed vitality. Hyaluronic acid fillers are also effective for intimate area rejuvenation, but their results are shorter-lived and less stable over time, often requiring repeat treatments. A good candidate for labiaplasty is primarily concerned with the size, shape, and symmetry of her labia. She is in good overall health, does not smoke, and has realistic expectations for the outcome of the procedure.

Prominent labia can also make certain physical activities uncomfortable, such as cycling, or make wearing tight clothing like swimsuits or fitness leggings difficult, especially for active women. The decision to undergo this procedure is often influenced by psychological factors. How you perceive your body can profoundly impact your self-image. Recovery is an important stage of any surgery. It is essential to schedule labiaplasty at a time when you can devote sufficient attention and rest to the healing process. Although recovery is generally straightforward, you must allow yourself to take it easy in the first few days. Some initial swelling may occur due to tissue trauma from surgery, which is often managed with anti-inflammatory and pain-relief medications. You can usually return to work and do other light activities within a week. However, vigorous exercise and intense physical activity should be avoided for 6 weeks. Intercourse and the use of tampons can typically be resumed after about 4 weeks. This procedure often provides long-lasting results and can lead to significant improvements in your overall quality of life.   

The area will be swollen immediately after the procedure, and you may experience some discomfort for 2-3 days. During the initial recovery period, it is important to rest. You should avoid intercourse, high temperatures, and vigorous exercise for 6 weeks after surgery. Some bruising may occur, but it typically resolves within a couple of weeks. Gentle skin massage, a balanced diet, proper hydration, and avoiding nicotine-based products all help speed recovery and ensure the best possible end result. The final appearance will not be immediately visible and requires a few months to settle, although this process is typically faster than in other areas.

On the day of your procedure, you will be welcomed into a room designed for your safety and comfort. After a preoperative evaluation with Dr. Elisa Bolletta, you will receive either local or general anesthesia, depending on the planned procedure. We will guide you through each step, explaining everything clearly to help ease any anxiety or uncertainty.

The duration of the procedure varies depending on its complexity and the technique used. A standard labiaplasty typically takes about 60 to 90 minutes, while genital fat grafting may take longer if combined with liposuction to harvest the fat.

Thanks to anesthesia, you will not feel any pain during the procedure. After surgery, it is normal to experience mild discomfort or tightness in the treated area. We will give you clear guidance on using appropriate pain medication to ensure your recovery is as comfortable as possible.

It is important to keep the treated area clean and follow all postoperative instructions carefully. Avoid sexual activity and strenuous exercise until fully healed, wear loose, comfortable clothing, and monitor the area daily for any signs of irritation or infection.

As with any surgical procedure, there are some risks, including infection, bleeding, imperfect scarring, or asymmetry. However, by following strict protocols and using precise surgical techniques, these risks are very rare. Choosing an experienced surgeon is essential to minimize them.

It is important to have realistic expectations. The procedure can enhance shape and symmetry, but full healing requires time and patience. Since everybody responds differently, careful postoperative care is key to achieving the best results.
