UNDERSTANDING THE PROCEDURE
Breast reduction surgery removes excess fatty tissue, glandular tissue, and skin to improve the appearance and proportions of your breasts while relieving discomfort caused by overly large breasts. Sometimes breast asymmetry and/or enlarged nipples are also present and can be corrected during the procedure. Whenever possible, I perform most breast reductions using a vertical approach based on the Hall-Findlay technique. This results in a lollipop-shaped scar around the areola that extends down to the inframammary fold. The nipple is not removed but repositioned upward, ensuring proper blood flow and circulation to the surrounding tissue. Excess skin and glandular tissue are then removed, and the remaining breast tissue is internally sutured to create an internal bra, resulting in a more compact and long-lasting shape.
My approach, based on scientific studies involving anthropometric measurements published in leading plastic and aesthetic surgery journals, allows me to reliably predict the medium- and long-term outcomes for this type of procedure.
All of this greatly helps me refine my surgical technique, allowing me to shape the breasts primarily using the glandular tissue rather than the overlying skin. The resulting scars are usually barely visible under a swimsuit or bra. I often find it beneficial to use liposuction in the lateral chest and underarm areas to contour the breasts further and enhance their final shape. In specific situations, such as with very large and sagging breasts, I perform breast reduction using an inverted T incision, adding a horizontal scar hidden in the inframammary fold, while in other cases, I rely solely on liposuction.
Ideal candidates for this procedure tend to be older patients with fattier and less glandular breast tissue. The surgical approach is determined by several factors, including the patient’s anatomy, the breast footprint on the chest wall, her aesthetic preferences, and the extent of reduction and desired volume. In the postoperative phase, a period of limited activity is necessary, during which the breasts may be swollen, bruised, and hypersensitive. Physical activity should be restricted and gradually resumed, especially when it involves strenuous upper-body movements.
Breast reduction surgery can provide long-lasting results, and many patients experience significant improvements in their quality of life. Smaller breasts help relieve pain and physical limitations, while also enhancing body image, with patients often feeling that their figure is slimmer and more balanced. However, I always remind patients that the procedure cannot guarantee permanent results. “The breasts will naturally continue to change with aging and may also be affected by weight fluctuations, hormonal factors, or pregnancy.”
FOR A MORE CONSCIOUS CHOICE
A good candidate for breast reduction is close to her ideal body weight, in good overall health, and has realistic expectations for the outcome of the procedure. Women with reduced skin elasticity or who have experienced significant weight loss may require longer incisions. Smokers must stop several weeks before surgery, as nicotine can impair blood flow, hinder healing, and affect the viability of the nipple-areola complex. Age is rarely a limiting factor for breast reduction surgery. I perform reduction procedures on patients aged 14 to 80. If a teenager is close to her ideal body weight, surgery may be appropriate, as weight loss does not typically affect breast size. For older patients, I assess suitability based on overall health rather than age.
Excessively large breasts can cause discomfort not only physically but also emotionally. Breast reduction surgery can be highly effective, creating a more balanced and shapely contour while restoring quality of life and confidence in your body. Self-esteem is an important consideration and should never be underestimated. For some women, large breasts can cause anxiety, embarrassment, and shame. Patients who have undergone breast reduction surgery sometimes report feeling self-conscious in social situations or during intimacy, and that the size of their breasts negatively affected their ability to achieve professional and personal goals. Others have shared that the unwanted attention their breasts attracted made them feel embarrassed or even depressed. Patients are often delighted to find that many common symptoms, such as back, neck, and shoulder pain, as well as neuropathic pain and headaches, completely resolve. During the consultation, I always explain that the final shape of the breast is determined by its original form, which guides how much tissue can be safely removed to achieve a balanced and natural result. Both physical and mental health play an important role in the success of breast reduction surgery. In some cases, it may be best to postpone the procedure until you have reached your ideal weight and are in optimal health. It is also important to plan your surgery around your personal and professional schedule. Keep in mind that you will need about 2 weeks before returning to your regular daily activities, and around 3-4 weeks before resuming your full routine, including physical exercise. If you have young children, I recommend avoiding physical exertion for at least the first 3 weeks after surgery, until you have fully regained your energy. During the consultation, I will remind you that breasts naturally change during pregnancy and breastfeeding, which could reverse the results of surgery. In some cases, particularly with extensive reductions, the procedure may also impact your ability to breastfeed. If you plan to have children in the future and breastfeeding is important to you, I recommend carefully considering the best time to undergo the procedure to ensure long-lasting and optimal results.
The final results will not be visible immediately. During the first 72 hours, swelling is normal. The breast tissue has not yet settled, and the breasts have not acquired their final shape. After 6 weeks, the initial sutures begin to dissolve, and the breasts will start to settle into place. Between 3 to 6 months post-surgery, the shape of your breasts becomes well-defined, and after 2 years, the scar tissue will have softened, giving your breasts an even more natural appearance that blends harmoniously with your body. Depending on individual needs, I often recommend a course of lymphatic drainage massages to reduce any residual fluid and soften the tissues, helping achieve the most natural result possible. I also firmly believe in postural reeducation and restoring muscle function by improving mobility in the chest and shoulders, supported by qualified physiotherapists and osteopaths who can guide your recovery safely and effectively. Postural reeducation can help correct any imbalances caused by the excessive weight of your breasts before surgery, preventing back and neck pain as well as potential muscle tightness.
On the day of your surgery, you will be personally welcomed to the clinic and guided by our team. After preparation and a pre-operative briefing with the doctor, the surgical plan will be carefully marked on your body, and the procedure will take place in the operating room under general anesthesia. Throughout the entire process, you will be continuously monitored to ensure complete safety and comfort. After the procedure, you will spend a period in recovery before returning to your room.
The duration of the procedure depends on its complexity and the amount of tissue to be removed, usually ranging from 2 to 4 hours. Surgeries involving the correction of asymmetries or significant volume reduction may take slightly longer.

After 4-6 weeks, and following your personalized instructions, you can gradually return to more demanding activities and exercise. A gradual, targeted approach is key to returning to your routine safely, comfortably, and smoothly.
After the procedure, a regenerative dressing will be applied to support tissue recovery. During the first few days, it is important to avoid strenuous activity, excessive sweating, and sudden movements of the chest and arms. During your recovery, be sure to wear the recommended post-surgical bra at all times, protect your breasts from any bumps or pressure, and carefully follow the instructions for your dressings. Bruising, swelling, and temporary changes in sensation are normal and will gradually resolve. Sleeping in a supine or semi-supine position promotes optimal healing. Once the wounds are dry, fully healed, and well-closed, the scars can be treated with specialized products containing silicone or exosomes, following the protocol prescribed by Dr. Elisa Bolletta. Keep in mind that the results evolve over time. Initially, the breasts may appear fuller and more lifted, gradually softening and taking on a natural shape over approximately four months.
As with any surgical procedure, there are risks, including bleeding, infection, visible scarring, changes in nipple sensation, residual asymmetry, or delayed healing of the incisions. Serious complications are rare, and our team works diligently to minimize them.

Follow-up care is ongoing and fully personalized. Your first check-up will take place 1-2 days after surgery, followed by appointments at 1 week, 1 month, 3 months, 6 months, and then annually. During these visits, we monitor your scars, breast shape, and overall comfort. The final visit, typically 1 year after surgery, allows us to assess the fully matured scar and the settled results, showing their natural, final appearance.
