UNDERSTANDING THE PROCEDURE
This procedure is becoming increasingly popular in plastic surgery, and its applications are rapidly expanding worldwide. For many years, I have made fat grafting a core part of my practice, both in aesthetic procedures and in reconstructive surgery, as an adjunct to implant-based reconstruction or following a lumpectomy.
he technique involves performing cosmetic liposuction on donor areas where fat is most abundant, such as the abdomen, hips, and thighs, using fine cannulas. The fat graft is carefully prepared and processed to remove impurities, including dead cells, fluids, enzymes, free radicals, and other residues. The purified fat is then reinjected using a retrograde technique with fine cannulas, allowing it to be deposited within the tissue.
The most critical aspect of the procedure is the meticulous reinjection, where thin ribbons of fat are placed precisely where needed, allowing me to smoothly volumize the tissue without creating lumps.
I consider fat grafting highly valuable for filling, shaping, and harmonizing the upper breast quadrants, the décolletage, and the breast projection. It is often used in combination with breast augmentation, a breast lift, or breast reconstruction, especially for prepectoral reconstructions. In these cases, fat grafting can be combined with implants to add volume and thickness above the implant, significantly improving both the tactile and visual perception of the breast. “Patients who have undergone a lumpectomy can also benefit greatly from fat grafting, which replaces deficient breast tissue and improves texture, shape, and any scar-related retractions.”
FOR A MORE CONSCIOUS CHOICE
It goes without saying that to be a good candidate for fat grafting, you must have clearly defined donor fat areas. One of the major advantages of fat transfer is that it simultaneously enhances the contour of the areas where liposuction was performed. For women considering breast augmentation using only fat, it is important to have realistic expectations regarding the amount of volume that can be added. The ideal candidates are those with well-shaped breasts and good skin quality who wish to increase their volume by one or two cup sizes. Women seeking a larger increase or who have sagging breast tissue may need to consider implants or combined procedures. Successful integration of the fat into the recipient site takes several months, and it is important to keep in mind that roughly 30% of the transferred fat may be reabsorbed over time. In reconstructive cases, the best candidates are women who, after immediate or delayed breast reconstruction with implants, experience the so-called rippling effect, which are folds and wrinkles in the upper and outer breast quadrants caused by thin residual tissue following mastectomy. Fat grafting softens the tissue and minimizes the sensation of a foreign body from the implant, providing a significant psychological boost. Even after breast-conserving surgeries, such as quadrantectomies or reshaping procedures followed by radiotherapy, fat transfer can help restore breast volume, enhance overall shape, and soften post-surgical scar tissue that affects the breast contour.
Fat grafting can be a great option for women who wish to avoid breast implants, since the tissue is entirely natural and requires no ongoing “maintenance.” However, there are limits to what this procedure can achieve. The length of the procedure varies depending on the extent of the fat grafting and the donor areas involved. Typically, it takes between 1.5 and 3 hours. When combined with other procedures, such as a breast lift or augmentation, the overall time will naturally be longer.
Fat grafting is generally well tolerated. Thanks to anesthesia, you will not feel any pain during the procedure. In the days following surgery, you may notice some tightness or bruising in the treated areas, which can be easily managed with prescribed pain medication. After the procedure, you will be asked to wear a compression garment or compression stockings in the fat donor areas for approximately 40 days.
Initial recovery typically takes about 7-10 days, during which comfort and bruising gradually improve. Light activities can usually be resumed after 5-7 days, while sports or more intense physical exercise should be restarted gradually after 3-4 weeks. The donor area may feel sore for a few days and show mild swelling or bruising, which typically resolves over time, often with the support of targeted therapies.

After the procedure, a regenerative dressing will be applied to support tissue recovery. Once the wounds are dry, fully healed, and well-closed, the scars can be treated with specialized products containing silicone or exosomes, following the protocol prescribed. Keep in mind that the results evolve over time. Initially, the breasts may appear fuller and more lifted, gradually softening and taking on a natural shape over approximately four months.
As with any surgery, there are potential risks, including:
• Skin irregularities or breast asymmetry
• Partial resorption of the fat tissue, which may require touch-ups
• Infections or hematomas in the treated areas
• Temporary bruising and swelling
Thanks to precise techniques and well-established protocols, the risk of significant complications is very low.
