AREOLA AND NIPPLE RECONSTRUCTION
I consider the reconstruction of the areola and nipple an integral and essential part of the breast reconstruction process. It restores a feminine, natural appearance to the reconstructed breast, reduces the risk of complications and asymmetry, and provides a powerful psychological benefit by helping the patient move beyond the memory of the disease.
Regardless of the technique used, it is essential to wait approximately 8 to 10 months after the breast reconstruction to allow the tissues to stabilize. This will ensure that the newly created areola and nipple are aligned, symmetrical, and at the correct height.
“The choice of technique depends on several factors, including the projection of your reconstructed breast and the characteristics of the areola and nipple on the opposite breast, such as color and size.”
When performing nipple reconstruction, I generally use your own tissue flaps, or a portion of the nipple from the opposite breast if it is large, or composite grafts, which can include small pieces of vulvar tissue, earlobe, or fingertip tissue. For areola reconstruction, I typically use medical tattooing, or in selected cases, I may take a small skin graft from the opposite areola or from other areas of the body (outer labia, behind the ear, upper eyelids, inner gluteal fold, or upper-inner thigh). Sometimes the opposite areola is also tattooed to ensure a perfectly even color. Reconstructing the areola and nipple completes the breast reconstruction journey and plays a vital role in your emotional and social recovery, helping you feel whole again.
UNDERSTANDING THE PROCEDURE
I usually reconstruct the nipple using small tissue flaps taken from the surrounding area and following the existing mastectomy scars. Meanwhile, for the areola, I prefer to use 3D medical tattooing.
The flaps are rotated and sutured to recreate the look and projection of a new nipple. In some cases, when the opposite nipple is large, I may take a tissue graft from it and transfer it to the breast being reconstructed.
For areola reconstruction, advanced medical tattooing techniques allow me to achieve a 3D effect that recreates the desired shape, color, and size. Drawing on my extensive experience and hands-on training, I use inorganic, hypoallergenic, and biocompatible pigments, carefully blending them and implanting them into the surface layer of the skin. The outcome is strikingly natural and mirrors your original look.
This procedure often requires a second touch-up after several months or years, as the medical tattoo may experience gradual pigment fading, and the nipple’s projection can diminish over time. “To address this, new techniques for nipple reconstruction have been developed in recent years, including the use of a biocompatible, perforated silicone implant known as Fix Nip, which is placed just beneath the skin through a tiny incision.”
FOR A MORE CONSCIOUS CHOICE
If your preoperative or intraoperative oncological condition required removal of the nipple-areola complex during the mastectomy, or if this area later developed necrosis or vascular complications leading to delayed healing, it is essential to plan your nipple-areola reconstruction at the appropriate time. Whenever possible, Dr. Elisa Bolletta prefers to use your existing scars to make them less visible and to restore greater harmony and natural balance to your breast.
Today, breast reconstruction surgery increasingly emphasizes aesthetic outcomes alongside oncological and reconstructive goals. This aesthetic approach, allows us to restore an essential part of your femininity: the nipple-areola complex. The possibility of preserving your nipple is often a reassuring thought during such a challenging time. Unfortunately, this is not always possible, as the disease may directly involve the nipple or intraoperative histological evaluations may require its removal. In these cases, Dr. Elisa Bolletta will explain that reconstructing this area is an important step not to be overlooked, as it helps you fully regain your physical and emotional well-being. While some external scars may never completely disappear, deeper emotional wounds can be eased by restoring an appearance that connects you to the version of yourself before your difficult battle with illness.
It is important to determine the most appropriate timing for your nipple-areola complex reconstruction. If you are undergoing oncological treatments, your clinical history and an oncologist’s input should be considered to decide whether to postpone the reconstruction to protect your skin and support proper healing. From a cosmetic perspective, it is also important to wait until your breast reconstruction results are stable and fully settled, which usually occurs around 8-10 months after the initial surgery. This allows for any necessary adjustments to achieve optimal symmetry. In the meantime, innovative silicone nipple sets are available. These soft, self-adhesive nipples come in a variety of shades and can be used to recreate a natural-looking nipple-areola complex during this transitional and often challenging period.
After reconstruction, the new nipple requires about 15 days to heal, after which the sutures are removed. For the following 4-6 months, you will need to use a silicone nipple protector to prevent any pressure on the treated area. It is common for some of the initial projection to gradually decrease during the first twelve months compared with the original projection. During the first 15 days, the tattooed area should be treated like a superficial wound, with gentle daily cleansing and the application of special soothing and regenerating creams, which will be prescribed.
On the day of your procedure, you will be welcomed by the team at Elisà Plastic Surgery in a calm and safe environment. After the final preoperative evaluations with Dr. Elisa Bolletta, local anesthesia will be administered, and the procedure will be performed according to the chosen technique, carefully planned to suit your individual characteristics and personal goals.
The procedure is generally brief, lasting about 1-2 hours, depending on the technique used for the reconstruction of the nipple-areola complex.
The procedure is performed under local anesthesia or sedation, so you will not feel any pain. After surgery, any discomfort is usually mild and well managed with appropriate medication.
Recovery is generally quick. Within a few days, you can resume normal daily activities while taking care to avoid any pressure on the treated area. Achieving a stable, lasting result takes several months, during which the tissues settle, and the color of the medical tattoo naturally harmonizes.
During recovery, it is essential to protect the area, avoid any trauma, and follow all instructions carefully. When a medical tattoo is performed, it is essential to meticulously cleanse and moisturize the skin with the recommended regenerative products.
This is a safe procedure, but it can carry some risks, such as gradual fading of the tattoo or partial loss of projection of the reconstructed nipple. In rare cases, infections or irregular scarring may occur.
For Dr. Elisa Bolletta, reconstructing the areola and nipple is a crucial step, marking the final stage of the breast reconstruction journey. This procedure restores a natural appearance, helps correct asymmetries, and has a profoundly positive psychological impact. It is important to note that, in some cases, a touch-up may be needed over time to maintain the best possible result.
After the procedure, regular follow-up visits are scheduled to monitor healing and evaluate the aesthetic results. If a medical tattoo has been performed, a review appointment will be arranged to assess color stability and determine if any touch-ups are necessary.
